💕 Key Responsibilities
As a Population Health Nurse, you will:
- 🌈 Champion inclusive care models that recognize and address health disparities among marginalized groups
- 📊 Assess population health trends to identify emerging risks and opportunities for intervention
- 📞 Deliver remote coaching and education for individuals managing ongoing health needs, including cardiovascular, metabolic, and respiratory challenges

📃 Required Qualifications
To succeed in this position, candidates should meet the following requirements:
- 🎓 Active Registered Nurse (RN) license in good standing; multi-state license is a plus
- 📆 Minimum 3 years of direct nursing experience, with at least 1 year in population health, care management, or public health
- 💻 Proficient in electronic medical records (EMR), telehealth platforms, and digital health tools
- 📈 Deep understanding of value-based care models and social determinants of health
- 💬 Excellent communication skills—both written and spoken
