About the Role: We seek an experienced and compassionate Telehealth Case Manager to join our team and provide high-quality virtual care coordination services. As a Telehealth Case Manager, you will be vital in assessing, planning, and coordinating healthcare services for patients using telehealth platforms.
Key Responsibilities:
- Client Assessment: Conduct comprehensive assessments of clients via telehealth to evaluate medical, psychological, and social needs.
- Care Planning: Develop and implement individualized care plans addressing a client’s health and wellness.
- Coordination of Services: Collaborate with healthcare providers, specialists, and community resources to arrange necessary treatments and support services for clients.
- Monitoring and Follow-Up: We regularly monitor client progress through virtual check-ins and update care plans to achieve the desired health outcomes.
- Education and Advocacy: Educate clients and their families on health conditions, treatment options, and available resources. Advocate for client needs to ensure optimal care delivery.
- Documentation: Maintain detailed, accurate, and timely documentation of all interactions and care coordination activities in compliance with organizational standards and healthcare regulations.
- Crisis Intervention: Provide immediate support and intervention during health-related crises, including connecting clients to emergency care services when required.
- Technology Utilization: Use telehealth software and tools efficiently to deliver seamless care and securely manage patient information.
- Compliance and Standards: We must adhere to all ethical, legal, and regulatory standards in telehealth and case management practices.
Qualifications and Skills:
- Bachelor’s degree in Nursing, Social Work, or a related healthcare field. Advanced degrees or certifications in case management are a plus.
- Minimum of 3 years of experience in case management, care coordination, or a related healthcare role, preferably with telehealth exposure.
- Strong knowledge of healthcare systems, community resources, and care management practices.
- Excellent communication skills, with the ability to engage effectively with clients, families, and multidisciplinary teams in a virtual setting.
- Proficiency in using telehealth platforms, electronic health records (EHR), and other digital tools.
- Compassionate, patient-focused approach with exceptional problem-solving and organizational abilities.
- Active licensure (e.g., RN, LCSW, or similar) in the state of residence, with the ability to obtain licensure in additional states if required.
