About the Role

As a Telehealth Nurse for Nonprofits, you will be critical in delivering accessible and patient-centered healthcare services to individuals and communities supported by nonprofit organizations. You will leverage telecommunication technologies to remotely provide patients with assessments, care plans, health education, and emotional support. This position combines clinical expertise, empathy, and technical proficiency to ensure the delivery of high-quality care in a virtual environment.

Key Responsibilities

  1. Patient Assessments and Care Planning
    • Conduct virtual consultations with patients to assess their medical conditions, health history, and concerns.
    • Develop personalized care plans and collaborate with other healthcare professionals to ensure coordinated care.
    • Monitor patient progress and adjust care plans to meet evolving health needs.
  2. Health Education and Counseling
    • Provide patients and caregivers with health education tailored to their conditions and needs.
    • Offer guidance on preventive care, chronic disease management, and medication adherence.
    • Support patients in understanding and navigating healthcare resources provided by nonprofit organizations.
  3. Telehealth Technology Utilization
    • Use telecommunication tools and platforms to deliver virtual nursing services effectively.
    • Ensure all interactions comply with telehealth protocols, HIPAA regulations, and organizational policies.
    • Troubleshoot technical issues and guide patients in accessing and using telehealth technologies.
  4. Collaboration and Advocacy
    • Work closely with nonprofit partners, community organizations, and healthcare providers to ensure comprehensive patient support.
    • Advocate for patients' needs and help connect them with additional resources and services.
    • Collaborate on public health initiatives and campaigns to promote wellness in nonprofit-supported communities.
  5. Documentation and Reporting
    • Maintain accurate and thorough electronic health records (EHRs) for all patient interactions.
    • Generate reports on patient outcomes, telehealth performance metrics, and areas for improvement.
    • Participate in regular team meetings and provide insights to improve telehealth programs.
  6. Quality Assurance and Continuous Improvement
    • Ensure that nursing care provided through telehealth meets the highest quality standards.
    • Stay updated on telehealth best practices, nursing trends, and changes in healthcare regulations.
    • Contribute to the development of telehealth protocols and policies to enhance service delivery.

Required Skills and Qualifications

  • Educational Background: Bachelor of Science in Nursing (BSN) is required. Advanced certifications in telehealth or public health nursing are preferred.
  • Licensure: Current and unrestricted RN license in the state of practice. Multistate or compact licensure is highly advantageous.
  • Experience: Minimum of 3 years of clinical nursing experience, with at least 1 year in telehealth, community health, or public health nursing.
  • Technical Proficiency: Familiarity with telehealth platforms, electronic health record (EHR) systems, and basic troubleshooting of digital tools.
  • Communication Skills: Excellent verbal and written communication skills to engage effectively with patients, caregivers, and team members.
  • Empathy and Compassion: A patient-centered approach that prioritizes understanding, respect, and emotional support.
  • Cultural Competence: Working sensitively and effectively with diverse populations and underserved communities.
