Key Responsibilities:

- Serve as the primary point of contact for patients, facilitating communication between patients and healthcare providers.
- Develop and maintain comprehensive care plans tailored to individual patient needs, ensuring all aspects of care are addressed.
- Schedule and manage virtual appointments, follow-up calls, and other patient interactions to support ongoing care.
- Care Management:
- Monitor patient progress and provide timely interventions to address any issues or concerns.
- Regular patient needs assessments should be conducted, and care plans should be adjusted to optimize outcomes.
- Ensure adherence to treatment protocols and guidelines by reviewing patient records and collaborating with healthcare providers.
- Patient Advocacy:
- Advocate for patients by addressing their concerns, answering questions, and providing support throughout their care experience.
- Educate patients about their conditions, treatment options, and self-care strategies to empower them in managing their health.
- Assist patients in navigating the healthcare system, including understanding insurance benefits and accessing necessary resources.
- Collaboration and Communication:
- Coordinate patient care by working closely with a multidisciplinary team of healthcare professionals, including physicians, nurses, and specialists.
- Facilitate communication between patients and their care team, ensuring all parties are informed and aligned on care plans and goals.
- Document and maintain accurate records of patient interactions, care plans, and progress notes securely and confidentially.
- Quality Improvement:
- Participate in quality improvement initiatives aimed at enhancing patient care and satisfaction.
- Analyze patient feedback and outcomes data to identify areas for improvement and implement necessary changes.
- Stay informed about best practices and emerging virtual care management trends to enhance service delivery continuously.
- Technical Proficiency:
- Utilize telehealth platforms and electronic health records (EHR) systems to manage patient information and facilitate virtual interactions.
- Troubleshoot technical issues related to virtual care tools and support patients as needed.
- Ensure compliance with data privacy and security regulations to protect patient information.
Qualifications:
- Education: Bachelor's degree in Nursing, Healthcare Administration, or a related field is required. A Master's degree is preferred.
- Experience: Minimum of 3 years of experience in patient care management, preferably in a virtual or remote setting.
- Certification: Relevant certifications, such as Certified Case Manager (CCM) or Certified Professional in Healthcare Quality (CPHQ), are highly desirable.
- Skills:
- Exceptional communication and interpersonal skills with the ability to build rapport and trust with patients.
- Strong organizational and time-management skills, with the ability to manage multiple tasks and prioritize effectively.
- Proficiency in using telehealth platforms, EHR systems, and other healthcare technologies.
- Analytical and problem-solving skills to address patient needs and implement effective care solutions.
